Health and Medicine

FSI’s researchers assess health and medicine through the lenses of economics, nutrition and politics. They’re studying and influencing public health policies of local and national governments and the roles that corporations and nongovernmental organizations play in providing health care around the world. Scholars look at how governance affects citizens’ health, how children’s health care access affects the aging process and how to improve children’s health in Guatemala and rural China. They want to know what it will take for people to cook more safely and breathe more easily in developing countries.

FSI professors investigate how lifestyles affect health. What good does gardening do for older Americans? What are the benefits of eating organic food or growing genetically modified rice in China? They study cost-effectiveness by examining programs like those aimed at preventing the spread of tuberculosis in Russian prisons. Policies that impact obesity and undernutrition are examined; as are the public health implications of limiting salt in processed foods and the role of smoking among men who work in Chinese factories. FSI health research looks at sweeping domestic policies like the Affordable Care Act and the role of foreign aid in affecting the price of HIV drugs in Africa.

In the midst of the damage to public health and the global economy, the COVID-19 crisis could present an unexpected opportunity both to resolve the only hot war in Europe and to address Russian President Vladimir Putin's assault on international norms of behavior.

As the spreading coronavirus and collapsing oil prices weigh increasingly on the Kremlin, the United States and its allies should offer to lift international sanctions against Russia if Putin will end his military incursions into Ukraine. President Trump and Congress can advance America's interests, and the world's, with a bold step to encourage an end to this war. The Trump administration and Congress should seize this opening.

As the COVID-19 pandemic continues to spread throughout the world, Japan is experiencing its second wave of coronavirus outbreak, following a first wave early on, just as it had become clear that the virus was spreading rapidly from Wuhan. In late February, travel restrictions were followed by Prime Minister Abe’s call for school closures. But as the pandemic raged through parts of Europe and then the United States, and as a growing number of countries issued shelter-in-place orders and lockdowns, Japan seemed relatively unscathed. Concerns then escalated and calls for voluntarily restricting peoples’ movement started in earnest following the decision to postpone the 2020 Olympics. On April 6, Prime Minister Abe declared a state of emergency for seven prefectures.

This panel brings together expertise on Japan’s political leadership with experience in Japan’s crisis management. Professor Harukata Takenaka has long studied how Japan’s political leadership has evolved, while Mr. Akihisa Shiozaki, an expert on crisis management, was a core member of Japan’s first private-sector investigative report after the Fukushima nuclear crisis.

This is the first in an APARC-wide series of virtual seminars that explore Asian countries’ responses to the COVID-19 pandemic. Held throughout the spring quarter, each event is led by one of APARC’s programs.

Akihisa Shiozaki, Partner, Nagashima Ohno & Tsunematsu

Akihisa Shiozaki is widely recognized for his expertise in corporate crisis management, including regulatory investigations, white collar defense, product recall, labor/employment disputes, import/export control, cyber security, media interaction and various corporate governance issues, especially those with multi-jurisdictional or parallel civil and criminal components. In recent years, he has advised both domestic and foreign clients in resolving a number of the most high profile corporate crises cases relating to Japan, including the LIBOR/TIBOR manipulation investigation, FX manipulation investigation, global product recall by a Japan auto-parts manufacturer, international trade secret theft in the semiconductor industry, government investigations against a global pharmaceutical corporation operating in Japan, and his representation of the former CEO of Olympus Corporation who brought light to the company's recent accounting scandal. He is recognized by Legal 500 as a Leading Individual in the field of Risk Management and Investigations. In 2017, Akihisa was awarded the Compliance / Investigations Lawyer of the Year at the Asian Legal Awards hosted by The American Lawyer, in association with The Asian Lawyer, China Law & Practice and Legal Week.

Akihisa worked in the Prime Minister’s office as senior policy advisor from 2006 to 2007 and is knowledgeable in Japanese regulations /rules and governmental procedures, as well as having rich experience dealing with the media. He also serves as the vice-chairman of the Anti-Yakuza Committee at the Daiichi Tokyo Bar Association and has authored many related publications. He graduated from the University of Tokyo (LL.B.), holds an M.A. in international policy from Stanford University, and completed his MBA at the Wharton School of the University of Pennsylvania where he served as class president.

Harukata Takenka, Professor of Political Science, National Graduate Institute for Policy Studies (GRIPS)

Harukata Takenaka is a professor of political science at the National Graduate Institute for Policy Studies in Tokyo.  He specializes in comparative politics and international political economy, with a particular focus on Japanese political economy. His research interests include democracy in Japan, and Japan's political and economic stagnation since the 1990s.  He received a B.A. from the Faculty of Law of the University of Tokyo and an M.A. and Ph.D. in political science from Stanford University.  He is the author of Failed Democratization in Prewar Japan: Breakdown of a Hybrid Regime, (Stanford University Press, 2014), and Sangiin to ha [What is House of Councillors], (Chuokoron Shinsha, 2010).

Kenji E. Kushida is a Japan Program Research Scholar at the Walter H. Shorenstein Asia-Pacific Research Center and an affiliated researcher at the Berkeley Roundtable on the International Economy. Kushida’s research interests are in the fields of comparative politics, political economy, and information technology. He has four streams of academic research and publication: political economy issues surrounding information technology such as Cloud Computing; institutional and governance structures of Japan’s Fukushima nuclear disaster; political strategies of foreign multinational corporations in Japan; and Japan’s political economic transformation since the 1990s. Kushida has written two general audience books in Japanese, entitled Biculturalism and the Japanese: Beyond English Linguistic Capabilities (Chuko Shinsho, 2006) and International Schools, an Introduction (Fusosha, 2008). Kushida holds a PhD in political science from the University of California, Berkeley. He received his MA in East Asian studies and BAs in economics and East Asian studies, all from Stanford University.

A policy brief by the Stanford Institute for Economic Policy Research (SIEPR) by several of our faculty members.

Even as people follow the rules of social distancing during the coronavirus pandemic, many still ask how they can best keep themselves safe when it comes to grocery shopping or being in situations where others are clustered. Should I wear gloves? And what about masks? Once businesses reopen and people head back to stores and restaurants, will wearing a medical mask still offer a smart level of protection?

The United States, Austria and some other countries outside of Asia — where mask use is already much more common — are now starting to consider recommending or are already requiring that people wear medical masks in public.

But the World Health Organization still recommends against wearing medical masks as long as you’re feeling well. The big reasons, they’ve argued, is that masks are ineffective and may increase risk to those who wear them incorrectly.

That recommendation deserves reconsideration, especially as policymakers think about what steps can be taken to ensure public safety while allowing more economic activity to resume.

What the Evidence Shows

Empirical evidence from existing observational studies and randomized trials supports the effectiveness of medical masks in reducing transmission of respiratory infections in a variety of settings.

Most observational studies, particularly around the SARS outbreak, have found mask wearing protects against infection. The evidence from randomized trials has been more mixed.

Several trials, conducted in community and health care settings, showed that wearing masks — when combined with thorough handwashing — proved to protect against respiratory infections, while other trials found no benefits. In many of these studies, less than 50 percent of participants actually used the protective measures, so that if more people take up these measures in the face of the pandemic, the benefit may be larger than what was found in the trials.

The most comprehensive reviews of the literature on the effectiveness of masks for interruption or reduction of the respiratory virus spread were conducted by Jefferson et al. in a 2011 Cochrane report and MacIntyre and Chughtai’s 2015 overview of the evidence.

Both reviews concluded that existing research supports the notion that having people wear masks makes good sense. Even if it can’t be measured with absolute certainty how much masks alone are responsible for cutting infection rates, there is no evidence to suggest that wearing a mask is dangerous for one’s health.  Both reviews acknowledge that the design, sample size, and analyses of the underlying studies are frequently non-ideal, limiting definitive conclusions on effectiveness. Nevertheless, the balance of evidence suggests a benefit of mask use in community settings, including among those who aren’t feeling sick, especially when masks are deployed early with respect to exposures and used in conjunction with other measures.

What Policymakers Can Do

As recently pointed out by some scientists following COVID-19, WHO’s recommendation against the use of masks by healthy people in community settings is based on the interpretation of this existing body of research as providing no evidence for the effectiveness of mask use in the community. The Centers for Disease Control and Prevention had followed a similar interpretation in the U.S., though both it and the WHO recommend the use of masks for symptomatic patients and health care professionals as effective means of preventing transmission.

While the quality of evidence supporting the effectiveness of masks in health care settings is certainly better, the firm recommendation against masks in community settings appears incompatible with the available evidence.

The strength of the recommendations against wearing masks appear to stem from two additional concerns: that the public would wear masks incorrectly, undermining their effectiveness; and that wide-spread community use would exacerbate mask shortages for health care professionals.

The World Health Organization has indicated that wearing masks incorrectly can increase one’s risk of infection. But there is not sufficient evidence to support the notion that people could not wear masks effectively. Many masks are packaged with detailed instructions for how to use them. And online videos could easily be posted to teach people how to wear a mask correctly. The theoretical risk of increasing acquisition of infection, while frequently cited by authorities, does not seem to be supported by a finding of increased risk in any of the available studies.

Appeals to the public not to stockpile masks so as to keep them available for health care professionals have generally not been effective, with widely reported stock-outs and shortages despite such appeals. We speculate that such appeals failed because the general public did not find the argument that masks are ineffective in community settings to be credible.

Moreover, it is possible that initial (and ongoing at the time of this writing) WHO and CDC guidelines against the use of masks in the community by individuals without symptoms may have unintentionally decreased the required sense of urgency and commitment of private and public resources for addressing underlying mask shortages for the general population. The result is that acute shortages of masks undercut even existing CDC recommendations; many individuals who are ill or visiting a health care facility with suspected COVID-19 symptoms cannot obtain masks.

Revising recommendations for expanding the use of masks in public areas in the U.S. is justified by the evidence. And guidelines to wear masks as part of other public efforts — including social distancing — to control the spread of COVID-19 could help steer the production energy and resources of both private and public players to ensuring there are enough masks for everyone.

Supporting the Economy

We further speculate that deployment of masks in public areas may eventually help the economy with transitioning into the post-COVID world.

First, masks could prove to be a cost-effective way of trying to reduce re-emergence of the virus in the medium run, as epidemiologic models suggest that virus re-emergence may necessitate re-introduction of social distancing measures with some regularity over the course of next 12 to 18 months (Flaxman et al. 2020). But wearing masks could give some hope of removing the need to put the economy into the switch on, switch off mode — which would require many businesses to close, open, and close again on short notice

Second, once official restrictions on many forms of economic activity are lifted, it is very likely that consumers will be hesitant in returning to their pre-pandemic levels of consumption of goods and services outside of their homes. Consumers’ behavioral sentiments are crucial for charting course of economic activity, while prolonged uncertainty may be harmful for firms (Bloom 2009).

Having a mask and knowing how to use it may serve as an extra crutch to help with inevitable consumer anxiety.

Jay Bhattacharya is a professor of medicine at Stanford Medicine and a core faculty member at Stanford Health Policy. His March 24, 2020, commentary in the Wall Street Journal questions the premise that “coronavirus would kill millions without shelter-in-place orders and quarantines.” In the article he suggests that “there’s little evidence to confirm that premise—and projections of the death toll could plausibly be orders of magnitude too high.”

In this edition of Uncommon Knowledge with Peter Robinson, the Murdoch Distinguished Policy Fellow at the Hoover Institution, Robinson asks  Bhattacharya to defend that statement and describe to us how he arrived at this conclusion. We get into the details of his research, which used data collected from hotspots around the world and his background as a doctor, a medical researcher, and an economist.

The Stanford Cyber Policy Center continues its online Zoom series: Digital Technology and Democracy, Security & Geopolitics in an Age of Coronavirus. These webinars will take place every other Wednesday at 10am PST.

In particular, Professor Broniatowski will discuss the results of new studies regarding bots and trolls in the vaccine debate, as well as what makes messages go viral from the standpoint of Fuzzy Trace TheoryProfessor Carley will explore how information moves from country to country, with a look at both the differences in who is broadcasting certain types of disinformation and the role bots play in the spread. Professor Shapiro will speak to trends and themes we are seeing in coronavirus disinformation narratives and in news reporting on COVID-related misinformation.


David Broniatowski 
Professor David Broniatowski conducts research in decision-making under risk, group decision-making, system architecture, and behavioral epidemiology. This research program draws upon a wide range of techniques including formal mathematical modeling, experimental design, automated text analysis and natural language processing, social and technical network analysis, and big data. Current projects include a text network analysis of transcripts from the US Food and Drug Administration's Circulatory Systems Advisory Panel meetings, a mathematical formalization of Fuzzy Trace Theory -- a leading theory of decision-making under risk, derivation of metrics for flexibility and controllability for complex engineered socio-technical systems, and using Twitter data to conduct surveillance of influenza infection and the resulting social response. 
Professor Kathleen M. Carley 
Professor Kathleen M. Carley is Director of the Center for Informed Democracy and Social-cybersecurity (IDeaS) and the director of the center for Computational Analysis of Social and Organizational Systems (CASOS). She specializes in network science, agent-based modeling, and text-mining within a complex socio-technical system, organizational and social theory framework. In her work, she examines how cognitive, social and institutional factors come together to impact individual, organizational and societal outcomes. Using this lens she has addressed a number of policy issues including counter-terrorism, human and narcotic trafficking, cyber and nuclear threat, organizational resilience and design, natural disaster preparedness, cyber threat in social media, and leadership.   
Professor Jacob N. Shapiro 
Professor Jacob N. Shapiro is professor of Politics and International Affairs at Princeton University and directs the Empirical Studies of Conflict Project, a multi-university consortium that compiles and analyzes micro-level data on politically motivated violence in countries around the world. His research covers conflict, economic development, and security policy. He is author of The Terrorist’s Dilemma: Managing Violent Covert Organizations and co-author of Small Wars, Big Data: The Information Revolution in Modern Conflict. His research has been published in broad range of academic and policy journals as well as a number of edited volumes. He has conducted field research and large-scale policy evaluations in Afghanistan, Colombia, India, and Pakistan.

Kelly BornKelly Born is the Executive Director of Stanford’s Cyber Policy Center, where she collaborates with the center’s program leaders to pioneer new lines of research, policy-oriented curriculum, policy workshops and executive education. Prior to joining Stanford, she helped to launch and lead The Madison Initiative at the William and Flora Hewlett Foundation, one of the largest philanthropic undertakings working to reduce polarization and improve U.S. democracy.  There, she designed and implemented strategies focused on money in politics, electoral reform, civic engagement and digital disinformation. Kelly earned a master’s degree in international policy from Stanford University.

To support Stanford students working in the area of contemporary Asia, the Shorenstein Asia-Pacific Research Center is offering up to ten research assistant internships for summer 2020 and up to three predoctoral fellowships for the 2020-21 academic year. The Center will review applications starting April 15 and expects to fill the positions by April 30, 2020. 

Amid the fallout of the COVID-19 pandemic, students are facing summer internship cancelations and hiring freezes. They are left wondering about the long-term implications of the current crisis for their academic careers and their access to future jobs and valuable work experience.

At Shorenstein APARC, we want to do all we can to support Stanford students. That’s why we are announcing new internship and fellowship opportunities for current students working in the area of contemporary Asia: research assistant internships for the summer quarter of 2020 and predoctoral fellowships for the 2020-21 academic year.

The summer internships are all remote: research assistants will work as telecommuters. Regarding the predoctoral fellowships, we hope we can all have a normal 2020-21 academic year, in which case we expect the fellows to be in residence, but we will reassess the evolving COVID-19 situation closer to the appointment start dates and shift to flexible, online options as needed.

APARC will review applications for both opportunities on a rolling basis starting April 15, 2020. The Center will select up to ten research assistants and up to three predoctoral fellows by April 30, 2020.

Summer 2020 APARC Research Assistant Internships

Shorenstein APARC is seeking highly motivated and dedicated undergraduate and graduate students to join our team as paid research assistant interns for the summer quarter of 2020. Research assistants will work with assigned APARC faculty members on projects focused on contemporary Asia, studying varied issues related to the politics, economies, populations, security, foreign policies, and international relations of the countries of the Asia-Pacific region.

All positions will be for eight weeks starting late June or early July 2020. The hourly pay rate is $17 for undergraduate students, $25 for graduate students.

Research assistant positions are open to current Stanford students only. Undergraduate- and graduate-level students are eligible to apply.

2020-21 Shorenstein APARC Predoctoral Fellowships

APARC is inviting applications from current Stanford students for the 2020-21 Shorenstein APARC Predoctoral Fellowship. The fellowship supports predoctoral students working within a broad range of topics related to contemporary Asia. 

Up to three fellowships are available to Ph.D. candidates who have completed all fieldwork and are nearing the completion of their dissertation. The Center will give priority to candidates who are prepared to finish their degree by the end of the 2020-21 academic year.

Shorenstein APARC offers a stipend of $36,075 for the 2020-21 academic year, plus Stanford's Terminal Graduate Registration (TGR) fee for three quarters. We expect fellows to remain in residence at the Center throughout the year and to participate in Center activities.

COVID-19 lockdowns have major impacts on migrant workers and supply chains that depend on them. Scott Rozelle and his team find that the lockdowns in China were successful in protecting rural areas from COVID infections, but that the cost was severe: Poor rural households cut down on education, nutrition, and health expenditures and lost around $100 billion in migrant worker wages.—Johan Swinnen, series co-editor and IFPRI Director General.

Maiko Ichihara is Associate Professor in the Graduate School of Law and the School of International and Public Policy at Hitotsubashi University, Japan, and a Visiting Scholar at the Center on Democracy, Development and the Rule of Law at Stanford University. She is a committee member of the World Movement for Democracy, East Asia Democracy Forum, and Partnership for Democratic Governance (Japan), and is a co-chair of Democracy for the Future project at the Japan Center for International Exchange. Throughout her career, she has undertaken research on international relations, democracy support, and Japanese foreign policy. She earned her Ph.D. in political science from the George Washington University and her M.A. from Columbia University. Her recent publications include: “Universality to Plurality? Values in Japanese Foreign Policy,” in Yoichi Funabashi and G. Speaker Bio:

Nancy Okail is a visiting scholar at CDDRL. She has 20 years of experience working on issues of democracy, rule of law, human rights, governance and security in the Middle East and North Africa region. She analyzes these issues and advocates in favor of human rights through testimony to legislative bodies, providing policy recommendations to senior government officials in the U.S. and Europe. She is currently the president of the board of advisors of The Tahrir Institue for Middle East Policy (TIMEP), previously she was the Institute’s executive director since its foundation. Dr. Okail was the director of Freedom House’s Egypt program. She has also worked with the Egyptian government as a senior evaluation officer of foreign aid and managed programs for several international organizations. Dr. Okail was one of the 43 nongovernmental organization workers convicted and sentenced to prison in a widely publicized 2012 case for allegedly using foreign funds to foment unrest in Egypt. She holds a Ph.D. from the University of Sussex in the UK; her doctoral research focused on the power relations of foreign aid.

Nathaniel Persily
Speaker Bio:

Nathaniel Persily is the James B. McClatchy Professor of Law at Stanford Law School, with appointments in the departments of Political Science, Communication and FSI.  Prior to joining Stanford, Professor Persily taught at Columbia and the University of Pennsylvania Law School, and as a visiting professor at Harvard, NYU, Princeton, the University of Amsterdam, and the University of Melbourne. Professor Persily’s scholarship and legal practice focus on American election law or what is sometimes called the “law of democracy,” which addresses issues such as voting rights, political parties, campaign finance, redistricting, and election administration. He has served as a special master or court-appointed expert to craft congressional or legislative districting plans for Georgia, Maryland, Connecticut, and New York, and as the Senior Research Director for the Presidential Commission on Election Administration. In addition to dozens of articles (many of which have been cited by the Supreme Court) on the legal regulation of political parties, issues surrounding the census and redistricting process, voting rights, and campaign finance reform, Professor Persily is also coauthor of the leading election law casebook, The Law of Democracy (Foundation Press, 5th ed., 2016), with Samuel Issacharoff, Pamela Karlan, and Richard Pildes. His current work, for which he has been honored as an Andrew Carnegie Fellow and a Fellow at the Center for Advanced Study in the Behavioral Sciences, examines the impact of changing technology on political communication, campaigns, and election administration.   He has edited several books, including Public Opinion and Constitutional Controversy (Oxford Press, 2008); The Health Care Case: The Supreme Court’s Decision and Its Implications (Oxford Press 2013); and Solutions to Political Polarization in America (Cambridge Press, 2015).   He received a B.A. and M.A. in political science from Yale (1992); a J.D. from Stanford (1998) where he was President of the Stanford Law Review, and a Ph.D. in political science from U.C. Nathaniel Persily is the James B. McClatchy Professor of Law at Stanford Law School, with appointments in the departments of Political Science, Communication, and FSI.  Prior to joining Stanford, Professor Persily taught at Columbia and the University of Pennsylvania Law School, and as a visiting professor at Harvard, NYU, Princeton, the University of Amsterdam, and the University of Melbourne. Professor Persily’s scholarship and legal practice focus on American election law or what is sometimes called the “law of democracy,” which addresses issues such as voting rights, political parties, campaign finance, redistricting, and election administration. He has served as a special master or court-appointed expert to craft congressional or legislative districting plans for Georgia, Maryland, Connecticut, New York, North Carolina, and Pennsylvania.  He also served as the Senior Research Director for the Presidential Commission on Election Administration. In addition to dozens of articles (many of which have been cited by the Supreme Court) on the legal regulation of political parties, issues surrounding the census and redistricting process, voting rights, and campaign finance reform, Professor Persily is coauthor of the leading election law casebook, The Law of Democracy (Foundation Press, 5th ed., 2016), with Samuel Issacharoff, Pamela Karlan, and Richard Pildes. His current work, for which he has been honored as a Guggenheim Fellow, Andrew Carnegie Fellow, and a Fellow at the Center for Advanced Study in the Behavioral Sciences, examines the impact of changing technology on political communication, campaigns, and election administration.  He is codirector of the Stanford Program on Democracy and the Internet, and Social Science One, a project to make available to the world’s research community privacy-protected Facebook data to study the impact of social media on democracy.  He is also a member of the American Academy of Arts and Sciences, and a commissioner on the Kofi Annan Commission on Elections and Democracy in the Digital Age.  Along with Professor Charles Stewart III, he recently founded HealthyElections.Org (the Stanford-MIT Healthy Elections Project) which aims to support local election officials in taking the necessary steps during the COVID-19 pandemic to provide safe voting options for the 2020 election. He received a B.A. and M.A. in political science from Yale (1992); a J.D. from Stanford (1998) where he was President of the Stanford Law Review, and a Ph.D. in political science from U.C. Berkeley in 2002.
