Nuclear energy is politically sensitive. For its proponents, nuclear energy is clean and highly efficient and indeed is the only alternative to fossil fuels in providing a base supply of electricity. For its opponents, nuclear energy is nothing but trouble, a symbol of war and weaponry par excellence, and one that creates environmental problems for mankind today and in the future. What is remarkable in this highly emotional debate is the general division between developed and developing countries. Asian and Gulf states are more active than many in other continents in expanding or developing their nuclear energy capacities. China is leading this expansion with 27 reactors under construction now.

Nuclear development in China highlights a series of objectives many developing countries try to balance – energy and economy, energy and development, energy and environment, energy and security, and the need for both clean energy and adequate and reliable energy supplies. It tells a counterintuitive story about Chinese politics – a single-party authoritarian political system with an extremely fragmented institutional structure in nuclear energy policy making, implementation and regulation and with highly competitive market forces in play. It provides a cautionary tale about the Chinese as well as global nuclear future. This paper discusses the challenges of nuclear energy development, using China as an example. It asks who drives it, what technology is selected and adopted, how human capital is developed, what the rules of the games are, and more importantly, which institutions are responsible for issuing licenses, regulating standards, and overseeing the compliance, and what forms of regulation do they use. At the core of these questions is if and how countries can ensure safe, secure and sustainable nuclear development.

Dr. Xu Yi-chong is a research professor of politics and public policy at Griffith University. Before joining Griffith University in January 2007, Xu was professor of political science at St Francis Xavier University in Nova Scotia, Canada. She is author of The Politics of Nuclear Energy in China (2010); Electricity Reform in China, India and Russia: The World Bank Template and the Politics of Power (2004); Powering China: Reforming the electric power industry in China (2002); co-author of Inside the World Bank: Exploding the Myth of the Monolithic Bank (with Patrick Weller 2009) and The Governance of World Trade: International Civil Servants and the GATT/WTO, (with Patrick Weller 2004); and editor of Nuclear Energy Development in Asia (2011) and The Political Economy of Sovereign Wealth Funds (2010). All these projects were supported by the research grants from either Social Sciences and Humanities Research Council of Canada (SSHRC) or Australian Research Council.

Michael McFaul is the former director of CDDRL and deputy director of FSI at Stanford University. He also is the Peter and Helen Bing Senior Fellow at the Hoover Institution, where he co-directs the Iran Democracy Project, as well as Professor of Political Science at Stanford University. 

Dr. McFaul is also a non-resident Senior Associate at the Carnegie Endowment for International Peace. He serves on the Board of Directors of the Eurasia Foundation, the Firebird Fund, Freedom House, the International Forum for Democratic Studies of the National Endowment for Democracy, and the International Research and Exchange Board (IREX).

He is the author and editor of several monographs including, with Anders Aslund, Revolution in Orange: The Origins of Ukraine's Democratic Breakthrough (2006) with Nikolai Petrov and Andrei Ryabov, Between Dictatorship and Democracy: Russian Postcommunist Political Reform (2004); with Kathryn Stoner Weiss, After the Collapse of Communism: Comparative Lessons of Transitions (2004); with Timothy Colton, Popular Choice and Managed Democracy: The Russian Elections of 1999 and 2000 (2003); Russia's Unfinished Revolution: Political Change from Gorbachev to Putin (2001); and with Tova Perlmutter, Privatization, Conversion and Enterprise Reform in Russia (1995). Professor McFaul comments frequently in the national media on American foreign policy and international politics. He has appeared on all major television and radio networks, while his opeds have appeared in The Chicago Tribune, The International Herald Tribune, The Los Angeles Times, The Moscow Times, The New Republic, The New York Times, The San Jose Mercury News, The Wall Street Journal, The Washington Post, The Washington Times and The Weekly Standard.

Dr. McFaul was born and raised in Montana. He received his B.A. in International Relations and Slavic Languages and his M.A. in Slavic and East European Studies from Stanford University in 1986. He was awarded a Rhodes scholarship to Oxford where he completed his Ph.D. in International Relations in 1991.

Dr. McFaul is currently on leave serving as the Special Assistant to the President and Senior Director for Russia and Eurasian Affairs at the National Security Council, where he is considered to be one of the top five national security players in government (The Washington Independent).

Kathryn Stoner-Weiss is a senior fellow at the Freeman Spogli Institute, deputy director of CDDRL, and faculty director of the Ford Dorsey Program in International Policy Studies at Stanford University. Prior to coming to Stanford, she was on the faculty for eight years in the Department of Politics and the Woodrow Wilson School at Princeton University. She also served previously as visiting associate professor of Political Science at Columbia University and assistant professor of political science at McGill University, Canada. Dr. Stoner-Weiss's research focuses on comparative state-building and effective governance, political economy of developing countries, and Russian domestic and international politics. She is the author of two books on Russian federalism and regional politics, as well as many books and articles on the same subject. She is also co-editor (with Michael McFaul) of After the Collapse of Communism: Comparative Lessons of Transition.

Kathryn Stoner is the Mosbacher Director of the Center on Democracy, Development, and the Rule of Law (CDDRL), and a Senior Fellow at CDDRL and the Center on International Security and Cooperation at FSI. From 2017 to 2021, she served as FSI's Deputy Director. She is Professor of Political Science (by courtesy) at Stanford and she teaches in the Department of Political Science, and in the Program on International Relations, as well as in the Ford Dorsey Master's in International Policy Program. She is also a Senior Fellow (by courtesy) at the Hoover Institution.

Prior to coming to Stanford in 2004, she was on the faculty at Princeton University for nine years, jointly appointed to the Department of Politics and the Princeton School for International and Public Affairs (formerly the Woodrow Wilson School). At Princeton she received the Ralph O. Glendinning Preceptorship awarded to outstanding junior faculty. She also served as a Visiting Associate Professor of Political Science at Columbia University, and an Assistant Professor of Political Science at McGill University. She has held fellowships at Harvard University as well as the Woodrow Wilson Center in Washington, DC. 

In addition to many articles and book chapters on contemporary Russia, she is the author or co-editor of six books: "Transitions to Democracy: A Comparative Perspective," written and edited with Michael A. McFaul (Johns Hopkins 2013);  "Autocracy and Democracy in the Post-Communist World," co-edited with Valerie Bunce and Michael A. McFaul (Cambridge, 2010);  "Resisting the State: Reform and Retrenchment in Post-Soviet Russia" (Cambridge, 2006); "After the Collapse of Communism: Comparative Lessons of Transitions" (Cambridge, 2004), coedited with Michael McFaul; and "Local Heroes: The Political Economy of Russian Regional" Governance (Princeton, 1997); and "Russia Resurrected: Its Power and Purpose in a New Global Order" (Oxford University Press, 2021).

She received a BA (1988) and MA (1989) in Political Science from the University of Toronto, and a PhD in Government from Harvard University (1995). In 2016 she was awarded an honorary doctorate from Iliad State University, Tbilisi, Republic of Georgia.

After 9/11, the administration of US President George W Bush initiated the era of the global war on terror. For many, this was a misguided response to terror attacks. But before the decade was over, US forces invaded two countries and are now fighting shadow wars in Yemen, Somalia, and Pakistan, while an air war continues in Libya. Pentagon commands cover the entire planet, and US military assistance programmes are active in almost every country.

 Wars reorder politics and values. They remake that which is taken to be true and right. They render the world unrecognisable from what it was when the balloon went up. That is why epochs of world history are so often marked off by the dates of wars. How should we understand the era of 9/11? In what historical timeline does it belong?

It is useful to begin by recalling some of what seemed true on September 10, 2001. The US enjoyed the unquestioned global supremacy of the "unipolar moment". The "end of history" beckoned, in which liberal democracy and free markets promised peace and prosperity everywhere for all time. The West and its international organisations managed the world, ultimately for the general good. Globalisation was bringing people closer together.

Today, each of these verities lies broken. The failure to understand the 1990s and the significance of the end of the Cold War has left us unable to understand what has been happening in the decade since 9/11.

Renewed global military commitments have hastened an inevitable US decline. The unrestrained power of finance capital is wrecking economies and societies across the Western world. Europe lies prostrate, bereft of anything approaching serious leadership, mired in the divisive politics of austerity and racist, anti-immigrant populism. The great international institutions have sat on the sidelines of the crises roiling the world. The communications technologies that were supposed to lead to mutual understanding instead assist revolts and terrorism, rioters and financial speculators, when they are not being used by states to spy on their own citizens, or by corporations to increase the number of consumer products people desire.

How is it that the received wisdom about the nature of world politics was so badly wrong? What did we fail to see and why?

The great conceit that blinds us is the idea that the powerful make history just as they please. We are particularly prone to this error when thinking about international politics. Assisted by opinion columnists and think tank gurus, we tend to view the world from the perspective of decision makers in the great powers. What should the West do about Libya? How should the US respond to state failure in Yemen or the Iranian bomb? What should the G-20 do about the debt crisis?


'Like cowboys at the rodeo'

In the domestic politics of our own countries, it is easy for us to see that politicians are like cowboys at the rodeo: hanging on for dear life before a force of nature tramples them underfoot. The "bull" that throws them could be a long-simmering social crisis, a downturn of the business cycle, or some series of events over which no one exercises control. The skill of the politician determines how long they can hang on, but we are left in little doubt as to where the real power lies.

We are forced to confront the underlying social structures, historical legacies, and economic relations that determine our fates. Human agency, too, has its say, but it is not only that of the great leaders that matter. Ordinary people and the social and political movements they create can drive events and force the "policymakers" to respond. But fate usually works against human purposes. Rarely do either the powerful or those who resist them achieve quite what they intended.

A wonderful example of the impoverished vocabulary with which we think about international politics is the idea that "Reagan won the Cold War". The vast apparatus that is a modern state is reduced to its leader - "Thatcher", "Gorbachev". Agency, the power to shape events, rests firmly in the hands of this leader, who is located in the global North. A range of violent struggles, fought almost entirely in the global South, are subsumed under one term which denies there was even a war at all. Most of all, a tidy end - 1989 - is imagined in which, needless to say, the good guys won.

Epoch-defining dates like 1989 or 9/11 invoke various imagined histories and geographies. But too often the dates with which we order world politics are curiously Eurocentric. It is European exploration, the French revolution, a Congress in Vienna, and German invasions, for example, which mark out the globe's historical eras: 1492, 1789, 1815, 1914, 1939.

We are thus singularly unable to grasp the global histories and social relations that delivered us to 9/11. Within the conventional terms of analysis of international relations, it is almost impossible to see the great social, political and economic struggles between the global North and South that have driven modern world politics. European imperialism and the prodigious efforts to incorporate ever more peoples and places, ever more domains of life, into the capitalist world system lie at the origins of these global histories.

It is crucial to underscore that imperialism, capitalism and the modern world they together did not simply emanate from Europe. They were joint productions with the non-European world, albeit amid unequal power relations. Imperialism requires collaborators, while capital needs labour. The first factories were not built in England, but in the Caribbean, producing sugar with African slaves.

The World Wars gutted European imperial power and unleashed struggles for independence across the global South, led almost everywhere by the anti-imperialist left. For over forty years revolutionaries and insurgents, death squads and soldiers, carried on a deadly combat.

 

The fall of the left

The global significance of 1989, broadly speaking, was the defeat in both the North and the South of the political left, of those political movements that sought to replace, contain, or redirect the expansive energies of capital in accordance with humane values.

During the Cold War, Western powers had to maintain social welfare systems at home lest communism begin to look attractive. The Soviets, meanwhile, tried and failed to demonstrate that they too could produce washing machines, refrigerators and other consumer items. With the collapse of the USSR, neoliberalism was unleashed and could begin in earnest to do away with welfare states in the West. "Shock therapy" was delivered to the former Soviet bloc countries, while the debt crisis was used to control many economies in the global South. No longer did the West have to secure Third World allies with lavish aid.

The defeat of the left produced two outcomes which have defined the last twenty years and will continue to make history over the next twenty.

The end of the Cold War did not mean the end of the dire social consequences of unrestrained capitalism. The grievances, the injustices, the poverty, the anger, the continued reduction of everything human to the bottom line, to something that can be bought or sold, all this remained, even intensified. But now it was not the left that would make political lemonade out of these lemons, but the right; not communists, but religious fundamentalists, both Christian and Islamic. This is where the Tea Party belongs, feeding on the misdirected resentments of those devastated by unregulated capitalism.

The second outcome of 1989 is a dramatic increase in the political power of capital. Across the Western world, but most especially in the US, politicians are in hock to Big Money, while corporate media fundamentally shapes political debate.


Blinkered worldview

The problem with this, as Karl Marx would have told us, is that while capitalists know what is in the interest of their specific business, they are unable to cooperate for the good of the system as a whole. To maintain a capitalist society of a kind anyone would want to live in requires tremendous public investment and infrastructure; a neutral, professional and active civil service; and a strong framework of effective, lawful regulation.

Few capitalists want to pay taxes for all this, or subject their industries to significant regulation. Give capitalists as a class too much political power, and they will enfeeble government with their special interests, lobbyists, and kept politicians. The consequence is the drama currently on display: the self-destruction of the West and its economies. It is plainly obvious that Western societies are in dire need of modernisation, investment, and strategies for growth and employment. But the political forces that might fight for these have long since been vanquished. Anguished experts like Paul Krugman are left accurately to foretell a doom that the political systems of the West willfully do nothing to avoid.

It is this self-inflicted crisis that drives the timing of the scale-down of the global war on terror currently underway. After all, nation-building at home or abroad requires taxes. Rather than occupying countries in a "forward strategy for freedom", as Bush termed the invasion of Iraq, the war on terror will transform into the police, spy and special operations war Western liberals had originally called for in 2001.

Unifying the eras of the Cold War, the 1990s, and the decade following 9/11, are reinvigorated efforts to control the politics, economies and populations of the global South in the wake of the collapse of formal empire in 1945. Wars of tremendous folly and human cost have marked this entire period, in Korea, Vietnam, Algeria, Mozambique, and across Central and Latin America, among others. Bush's travesties in Iraq and Afghanistan will likely bring this series to an historical close. The tentative approach to Libya is a transition to a future of reduced Western ambition in the global South.

The retreat of the West from extensive and effective political and military efforts to control the global South - a history which began in the 16th century - rings the death knell of Western world power. The decade since 9/11 is the penultimate chapter in this history.

"The Fixer:The Taking of Ajmal Naqshbandi" is a feature-length documentary that follows the relationship between an Afghan interpreter and his client, American journalist Christian Parenti. This intimate portrait of two colleagues shifts dramatically when Ajmal is kidnapped along with an Italian reporter. The situation goes from bad to worse as foreign powers pressure for fast results, the Afghan government bungles its response and the specter of Taliban power looms in the background. What follows is the tragic story of one man forgotten in the crossfire: a brutal allegory of the proud land and perilous misadventure that is Afghanistan.

World War Two, the most violent period in the modern history of Europe and Asia (1937–1945), left deep scars still evident on both continents. Numerous and often conflicting narratives exist about the wartime era, ranging from personal memoirs to official accounts of wartime actions. Many issues, from collaboration to responsibility for war crimes, remain unresolved. In Europe some issues that have been buried for decades, such as the record of collaboration with Nazi occupiers, are now resurfacing. In Northeast Asia, World War Two’s complex, painful legacy continues to impact popular culture, education, diplomacy, and even economic relations.

While differences exist in the wartime circumstances and reconciliation processes of Europe and Asia, many valuable lessons can be gained through a study of the experiences on both continents. The Walter H. Shorenstein Asia-Pacific Research Center (Shorenstein APARC) facilitated a comparative dialogue on World War Two, bringing together 15 noted experts for the Colonialism, Collaboration, and Criminality conference, held June 16 to 17 at Stanford. Each of the event’s five panels paired an Asia and a Europe scholar addressing a common theme.

The debate over remembrance of World War Two

Asia’s relative lack of progress in achieving reconciliation of the painful legacies of the war in Asia and the Pacific continues to bedevil current relations in the region. This is a consequence of the way the Cold War interrupted the resolution of wartime issues and blocked dialogue over the past, particularly between Japan, China, and South Korea, suggested Daniel C. Sneider, associate director for research at Shorenstein APARC. The widely held image of an unrepentant Japan ignores the fierce debate within Japan over wartime memory, often obscured by the prominence of rightwing nationalist views. Meanwhile, within China and Korea, wartime memory is also increasingly contested ground, from the issue of collaboration to the emergence of a more nationalist narrative in China, further complicating relations among those Asian neighbors.

Daniel Chirot, a professor of international studies at the University of Washington, emphasized that immediate postwar economic and security needs, including the growth of Communism, accelerated West Germany’s willingness to reconcile with its Western neighbors. He concurred with Sneider, saying that no such imperative existed in Northeast Asia until the need for economic cooperation three decades after the war. He suggested that the growth of regional integration might, as in Europe, drive Northeast Asia toward greater reconciliation.

Divided memories

Justice for sensitive historical human rights issues, such as World War Two atrocities, bears increasing importance in today’s ever-globalized economic and political climate, stated Thomas Berger, a professor of international relations at Boston University. Berger noted the challenge that Japan’s factional politics poses to a revision of the country’s official wartime narrative, and suggested that a strong regional structure, such as the European Union, could effectively facilitate reconciliation in Northeast Asia.

Frances Gouda, a professor of political science at the University of Amsterdam, examined the use of Anne Frank and former Indonesian president Sukarno as “icons of memory” in Dutch interpretations of World War Two. She asserted that Frank’s victimization allows people to come to terms with Nazi war crimes, but that Sukarno’s vilification as a Japanese collaborator oversimplifies history and allows the Netherlands to avoid confronting its own colonial past.

Collaboration and resistance

France’s Vichy regime, responsible both for collaborating with the Nazis and acting independently to persecute Jewish citizens, remains a painful and unresolved subject in the country’s contemporary quest for national identity, said Julian Jackson, a professor of history at Queen Mary, University of London. He pointed to French president Nicolas Sarkozy’s act of making a national martyr out of Guy Môquet, a young communist who died resisting the German Occupation, as a key example of the complexities involved in trying to come to terms with France’s past.

Ongoing territorial disputes over islands located between Japan and its neighbors in China and Korea are a product of the unresolved legacy of the wartime era in Asia. Sovereignty over those islands was left deliberately unresolved by the 1951 San Francisco Peace Treaty which formally ended the war, suggested Alexis Dudden, a professor of history from the University of Connecticut. As a result, the territorial disputes have become a battleground on which larger questions of historical memory about the war are contested, not only by Japanese conservatives but also by Koreans and Chinese, she said.

Paths to reconciliation

Gi-Wook Shin, director of Shorenstein APARC and a professor of sociology, suggested that while Europe’s experience with war and reconciliation offers lessons for Asia, significant differences exist between the wartime and post-war situations of the two continents, and that reconciliation in Asia requires time. Increased economic interaction between the countries in Northeast Asia serves less to foster reconciliation, he said, than to spur competition for regional dominance. Shin emphasized that the United States, which has greatly impacted the region’s post-war history, can play a critical role as a facilitator in establishing lasting regional accord.

The Nazi regime’s systematic attempt to completely wipe out all traces of Jewish history and culture in Europe, even as closely bound as it was with Germany’s own traditions, is a unique case, stated Fania Oz-Salzberger, a professor of history at Haifa and Monash Universities. She explored universal elements in the German-Jewish reconciliation experience, noting, like Shin and Chirot, the important element of time that is needed to reflect upon painful events of the past. Oz-Salzberger especially spoke of the healing that takes place at the level of society and culture, sometimes even before governments are ready to reconcile with one another.

Continuing political impacts

Gilbert Rozman, a professor of sociology at Princeton University, suggested that Northeast Asia’s wartime history debates will continue to complicate regional relations unless China, Japan, and Korea reach a point of mutual reconciliation. He noted the role that Japan’s government, in the 1980s during its financial heyday, and more recently, China’s leaders during a similarly strong economic era, have played in prolonging the debate. 

Memories of war are transmitted across the years through a complex process involving multiple actors and they can later influence political behavior, explained MIT political science professor Roger Petersen. He described the process within the context of the Lithuania’s successful declaration of independence from the former Soviet Union in January 1991. Petersen stated that Lithuanian émigrés, in part, helped keep the narrative of Soviet aggression and Lithuanian martyrdom alive until the conditions were right for action many decades later.

The Colonialism, Collaboration, and Criminality conference grew out of Shorenstein APARC’s Divided Memories and Reconciliation project, which for the past three years has examined the legacy of war-era memories in Northeast Asia and the United States and explored possible means of reconciliation. Shorenstein APARC has already published the first in a series of four books based on the project, and an edited volume of papers from the June 2011 conference is forthcoming next year.

In spite of the economic advances and increases in GDP since the collapse of communism, Russia suffers from a range of dismal public health outcomes reminiscent of a much poorer country. This study seeks to understand what role political factors play in the country's high adult mortality rate and declining life expectancy by mining World Bank and World Health Organization data and examining how Russians access healthcare services and information.

In addition to many articles and book chapters on contemporary Russia, she is the author or co-editor of six books: "Transitions to Democracy: A Comparative Perspective," written and edited with Michael A. McFaul (Johns Hopkins 2013);  "Autocracy and Democracy in the Post-Communist World," co-edited with Valerie Bunce and Michael A. McFaul (Cambridge, 2010);  "Resisting the State: Reform and Retrenchment in Post-Soviet Russia" (Cambridge, 2006); "After the Collapse of Communism: Comparative Lessons of Transitions" (Cambridge, 2004), coedited with Michael McFaul; and "Local Heroes: The Political Economy of Russian Regional" Governance (Princeton, 1997); and "Russia Resurrected: Its Power and Purpose in a New Global Order" (Oxford University Press, 2021).

She received a BA (1988) and MA (1989) in Political Science from the University of Toronto, and a PhD in Government from Harvard University (1995). In 2016 she was awarded an honorary doctorate from Iliad State University, Tbilisi, Republic of Georgia.

Doctors have a long history of complicity in torture, but the torture of political dissidents holds a privileged place.  In Saddam Hussein's Iraq, surgeons removed the ears of men who failed to report for military service or defected from the army. In the Soviet Union, psychiatrists held political dissidents in mental hospitals with false diagnoses, in order to isolate and punish them. It is in this tradition of medical torture of dissidents that the Syrian healthcare establishment may be heading.

A July 6 report by Amnesty International documents the treatment of Wassim, a 21-year-old protester in the Syrian town of Talkalakh. After an injury from a soldier's bayonet, Wassim was taken to al-Bassel hospital, which had been occupied by Syrian security forces. As he reported: "The nurses, men and women […] swore at me and beat me hard and one female nurse punched me repeatedly with all her strength on my chest. Some were taking off their shoes and slapping me with them. I could hear many voices asking: 'You want freedom, eh?'" The report states he later had his wounds stitched without anesthesia, before being beaten on these wounds by hospital staff.  

Wassim's is not an isolated incident. In May, Reuters documented the case of a protester who had lost sensation in his legs who requested to see a doctor in jail. He told the news agency: "The doctor hit my knees with his legs, and asked: 'There, is it better now?' and then he slapped me". Most pervasively, reports suggest that even when doctors have not been involved in direct abuse, they have falsified the causes of injuries and released information about patients to the Syrian regime's security forces. The result is a public distrust of hospitals, and a clear incentive for injured protestors to avoid the healthcare system. 

The medical torture of political dissidents holds a privileged place because it can be perversely justified. The torture of dissidents may be seen as an act of loyalty to the state. Doctors acting on behalf of the state, such as military doctors, have what is called "dual loyalty" - loyalty to both their patient and a third party.

In addressing the issue of dual loyalty, Physicians for Human Rights has proposed guidelines that physicians not be present when torture takes place, and calls on them to report all human rights violations, especially when they interfere with their loyalty to patients. Like the medical professionals from the US recently implicated in the torture and abuse of prisoners at Guantánamo Bay and Iraq, some Syrian doctors may have valued their contribution to the security of the state more than their adherence to the norms of their profession. 

But, in their pursuit of perceived enemies of the state, have these physicians become enemies of the profession? Doctors involved in torture should be pursued as enemies of medicine: their crimes documented, their professional credentials revoked, and their ability to practice internationally thwarted.

Identifying and disqualifying doctors involved in torture

While it is exceedingly unlikely that Bashar al-Assad, an ophthalmologist, will go back to correcting cataracts in London - where he trained - if his regime is overthrown, other physicians culpable in his regime's torture will seek to continue clinical practice abroad.

Even with continued instability, it is likely that physicians and other elites will seek to emigrate. Could doctors involved in abuse head to Europe, North America or neighbouring Arab countries and continue to operate? How will they be identified? Critically, the majority of Syrian physicians that have not been complicit with abuses must be distinguished from those who have. 

Unfortunately, the medical profession has no method for identifying or punishing doctors complicit in torture. We rely on human rights organisations to provide sporadic documentation of medical torture.

With limited access and competing priorities - such as being able to provide medical care while working in countries where torture occurs - these organisations have a narrow scope for documenting the occurrence of torture. In an excellent Lancet article, Len Rubenstein and Melanie Bittle argue that the World Health Organization is best positioned to play a leading role in documenting attacks on medical functions in conflict, and this should include those attacks committed by physicians.

Among the suggestions put forth by Rubenstein and Bittle are a UN Security Council resolution providing a mandate for the WHO to pursue investigations, and the use of mobile devices for securely and quickly transmitting information about abuse. By documenting medical complicity in torture, we give physicians under incredible pressures incentive to oppose orders from their superiors and the state.

The greatest challenge, however, is enforcement, and the punishment of physicians complicit in torture. No international body retains information on professional qualifications. Like most other professions, medicine has proclaimed a need to be self-regulating, yet it has no system in place to disqualify or sanction physicians on a global level (national licensing bodies exist in most countries, but there is little to no international coordination). To this day, investigations continue of Rwandan doctors now practising in Europe and Africa, accused of involvement in the 1994 genocide.

Of course, their crimes were far more widespread than those in Syria today, as doctors oversaw the killing of hundreds of patients and staff in their hospitals, but the challenge of enforcement is nearly identical. Even if medical complicity in torture does not warrant imprisonment, it ought to warrant professional disqualification - and as of yet, no institution or process is in place to disqualify a physician from practising internationally. 

Honouring the heroism of Syrian doctors

Attacks on the healthcare system are common - perhaps inevitable - in modern war, but doctors don't always become complicit. In Bahrain, the Salmaniya medical centre was raided, and its doctors beaten and jailed for treating protesters. In Libya, Misurata hospital came under fire, deterring the sick from seeking care and endangering staff and patients.

Despicable as these attacks are, they have come to be expected as a feature of conflict. Attacks on the healthcare system have been documented in almost all recent conflicts including in Afghanistan, Kosovo, Nepal, Iraq, and the occupied Palestinian territories. In most cases, doctors have acted admirably, and sometimes heroically: seeing the sick in their homes, in secretive and makeshift clinics, risking their lives to provide care. Under oppressive regimes, doctors may be risking their lives just by refusing to be complicit in torture. 

In Syria, a group known as the "Damascus Doctors" has been organising on Facebook to provide hidden clinics in areas of protest, as reported by CNN. These doctors are upholding a tradition of professionalism and protest that existed since at least 1980, when more than 100 healthcare professionals were arrested for striking to demand the lifting of Syria's state of emergency, in place since 1963 (as of 1990, at least 90 of them remained missing). These doctors, like many others who have opposed the regime, were subjected to gruesome physical and psychological torture. 

The overwhelming majority of Syrian physicians have likely been acting heroically. It is in their honour that we should pursue aggressive international efforts to document and disqualify those physicians complicit in torture. This will require emboldened international institutions, cooperation among national licensing bodies, and the courage of doctors, journalists, activists and human rights organisations in documenting and reporting medical torture.
