Objective: To study when and how an urgent public health message about a boil-water order reached an urban population after the Massachusetts water main break. Methods: In-person surveys were conducted in waiting areas of clinics and emergency departments at a large urban safety net hospital within 1 week of the event. Results: Of 533 respondents, 97% were aware of the order; 34% of those who lived in affected cities or towns were potentially exposed to contaminated water. Among those who were aware, 98% took action. Respondents first received the message through word of mouth (33%), television (25%), cellular telephone calls (20%), landline calls (10%), and other modes of communication (12%). In multivariate analyses, foreign-born respondents and those who lived outside the city of Boston had a higher risk of exposure to contaminated water. New modes (eg, cellular telephones) were used more commonly by females and younger individuals (ages 18 to 34). Individuals who did not speak English at home were more likely to receive the message through their personal networks. Conclusions: Given the increasing prevalence of cellular telephone use, public officials should encourage residents to register landline and cellular telephone for emergency alerts and must develop creative ways to reach immigrants and non-English-speaking groups quickly via personal networks. Abstract (provisional)

Objective

Few studies have examined the link between health system strength and important public health outcomes across nations. We examined the association between health system indicators and mortality rates.

Methods

We used mixed effects linear regression models to investigate the strength of association between outcome and explanatory variables, while accounting for geographic clustering of countries. We modelled infant mortality rate (IMR), child mortality rate (CMR), and maternal mortality rate (MMR) using 13 explanatory variables as outlined by the World Health Organization.

Results

Significant protective health system determinants related to IMR included higher physician density (adjusted rate ratio [aRR] 0.81; 95% Confidence Interval [CI] 0.71-0.91), higher sustainable access to water and sanitation (aRR 0.85; 95% CI 0.78- 0.93), and having a less corrupt government (aRR 0.57; 95% CI 0.40- 0.80). Out-of-pocket expenditures on health (aRR 1.29; 95% CI 1.03- 1.62) were a risk factor. The same four variables were significantly related to CMR after controlling for other variables. Protective determinants of MMR included access to water and sanitation (aRR 0.88; 95% CI 0.82- 0.94), having a less corrupt government (aRR 0.49; 95%; CI 0.36- 0.66), and higher total expenditures on health per capita (aRR 0.84; 95% CI 0.7 0.92). Higher fertility rates (aRR 2.85; 95% CI: 2.02- 4.00) were found to be a significant risk factor for MMR.

Conclusion

Several key measures of a health system predict mortality in infants, children, and maternal mortality rates at the national level. Improving access to water and sanitation and reducing corruption within the health sector should become priorities.

More than 500,000 people live in Mathare, the second-largest collection of slums in Nairobi, Kenya. Crime and disease ravage the population, and shanties have no electricity or running water. But there’s one piece of technology that everyone seems to have, one which promises to bring much-needed improvements: the cell phone.

Cell phones are central to two of the eight most recent Global Underdevelopment Action Fund projects funded by the Freeman Spogli Institute for International Studies. Both projects will be led by Joshua Cohen, a professor of political science, philosophy and law and the Marta Sutton Weeks Professor of Ethics in Society.

The first of Cohen’s projects will examine whether texting private and accurate health advice will increase awareness of risky sexual behavior among Mathare’s younger residents who face high rates of pregnancy and sexually transmitted diseases.

Cohen's second project involves teaching women to use mobile technology to meet up with larger groups while traveling at night, which Cohen believes can led to a decrease in the number of assaults, muggings and rapes. Eventually, the hope is that the program can be taken over by Kenya’s police and expanded.

Six other projects are receiving support from the third round of Global Underdevelopment Action Fund awards, amounting to a total of $265,000.

The funds will enable multidisciplinary teams led by Stanford faculty from across the university to perform policy-relevant research focused on global underdevelopment challenges.

The funded projects will have real world impact. They will help target tuberculosis, which kills more than 1 million people a year and hinders economic development in the hardest-hit regions, like parts of India. They evaluate the amount of resources necessary to improve test scores and lower anemia rates among China’s rural schoolchildren. They ensure that health care is accessible to people in the Arab world where countries are undergoing political transitions. And they evaluate the challenges and benefits of bringing solar power to areas in Africa where electricity is a rare commodity.

As varied as the eight projects are, each will train Stanford undergraduate or graduate students, stressing the importance that Stanford and FSI place on training the next generation of researchers and policy influencers.

The projects were selected by a faculty committee chaired by Stephen D. Krasner, FSI’s deputy director and the Graham H. Stuart Professor of International Relations.

The Action Fund is supported by FSI donors, matching funds from the Office of the President and FSI. The fund grew out of the institute’s 2010 conference on Technology, Governance, and Global Development. This year’s follow-up conference further showcased FSI’s commitment to challenges posed by global underdevelopment with a focus on food security and health.

  • Texting for Sexual Health: Effects of Information Provision and Common Knowledge on Health-Seeking Behavior in Kenya
    Joshua Cohen
    In hopes of increasing awareness that could minimize sexual health risks, the team will promote a mobile health counseling service, which will enable young people in Nairobi’s Mathare slums to receive private and reliable answers from health counselors through text messaging.
  • Can Mobile Phones Coordinate Community Action to Improve Women’s Safety in Slums?
    Joshua Cohen
    The program uses mobile technology to measure whether the number of assaults on women will decrease if they travel in groups. The project will evaluate whether this “safety in numbers” program can be taken over by Kenyan police.
  • Crime, Violence and Governance in Latin America: Sharing Data and Building a Web-Based Research Network to Expand Knowledge
    Mariano-Florentino Cuéllar
    This project will build a systematically organized repository of research on crime, violence and citizen security in Latin America.

When it comes to climate change and its impacts on agriculture, we may know less than we think.

But according to David Lobell, Assistant Professor in Stanford’s Department of Environmental Earth System Science, acknowledging the gaps in our understanding could help us to more effectively prepare the world’s food system for a warmer future.

Lobell, who has built an impressive career around the study of climate change and its implications for global food security, addressed the topic of agricultural adaptation during a two-hour symposium held on the Stanford campus in early December. His presentation summarized the strengths and weaknesses of climate models in the context of global agriculture, and suggested broad strategies for preparing agriculture for climate change’s inevitable impacts.

Lobell began his talk by reaffirming some common beliefs. The Earth as a whole is unquestionably warming, he said. Precipitation intensity is increasing in high-rainfall areas, and the world’s driest regions are becoming drier.

“Think about the hottest day we currently experience in a 20-year period,” Lobell told listeners. “By mid-century, we’ll be seeing that hottest day every year, as opposed to every 20 years.” During the same period, soil moisture content in many of the world’s major agricultural areas will decrease by as much as 10 to 15 percent, while annual precipitation at the equator and high latitudes will increase by several inches per year.

At the global scale, Lobell said, climate change will have a net negative impact on existing agricultural systems. The world’s rainfed farms will become increasingly vulnerable to heat and water stress.  Growing ranges and seasons for heat-intolerant crops, such as wheat and sorghum, will contract. Although the high latitudes may see some gains from warmer temperatures and CO2 fertilization of certain crops, low-latitude regions – including South Asia and much of Africa – will suffer disproportionate yield losses as temperatures rise.

However, Lobell said that impacts aimed at local and national scales, as opposed to broad regions or the world as a whole, are much more difficult to predict. A moderate change in average rainfall across a continent could translate to drastic increases or decreases in individual countries. For example, while climate models suggest that Africa’s annual rainfall will change by less than 10 percent over the next 50 years, model projections show rainfall in the nation of Sengal changing by anywhere from five to 40 percent over the same period.

Additionally, Lobell said, forecasts of increasing climate variability are frequently overstated. “The number one misperception I hear is that climate change is going to mean more variability,” he noted.  In fact, model projections of year-to-year variability in temperature and precipitation cover a wide range. Some models do show large increases in variability over the next century – but others show a slight decrease.

Because we understand climate impacts best at the long-term and global scales, Lobell said, global responses that address long-term trends are the most likely to serve our future needs. He cautioned against approaches that prepare farmers for short-term variability, such as sudden floods or droughts, but fail to acknowledging the effects of steadily rising average temperatures. He also stressed the value of globally coordinated efforts, particularly those aimed at developing better heat and drought-tolerant crop varieties, to supplement local infrastructure projects.

 “We’re in a world where local resilience depends on global systems,” Lobell noted. He said that the interconnectedness of modern global food markets makes global trends, and global responses, increasingly relevant for local food security.

At both local and global levels, an effective response to climate change will require robust social institutions. Dr. Fatima Denton, Program Leader for Climate Change Adaptation in Africa for the Consultative Group on International Agricultural research, stressed this point in her comments on Lobell’s presentation. “Climate change has really unmasked our governance challenges and the weaknesses in our institutions,” Denton said. “This is not just about biophysical processes…it’s about the development pathways that we choose.”

Lobell agreed. Climate change, he said, presents “an important opportunity for transformation.” He encouraged present and future leaders to think critically about all aspects of the relevant science and policy. “Be skeptical of what you hear,” he advised, “and educate yourself about what we do and don’t know.”

The National Council for Science and the Environment (NCSE) will present its 12th National Conference on Science, Policy and the Environment: Environment and SecurityJanuary 18-20, 2012 in Washington, DC.  

The Environment and Security Conference will provide a forum to explore the connections between environment and security issues, their common underlying scientific threads, and the policy and governance needed to address security risks posed by a rapidly changing environment.

The conference is expected to bring together over 1,000 attendees from the scientific, business, academic and environmental communities, as well as international, federal, and regional government officials

NCSE utilizes a multi-disciplinary and multi-sectoral approach to covene involved scientists and decision-makers from various sectors of society. The conferences include renowned speakers, topical symposia to explore issues more in depth, and breakout workshops to develop a set of recommendations on how to advance science and connect it to policy and decision-making.

Rosamond Naylor is the William Wrigley Professor in Earth System Science, a Senior Fellow at Stanford Woods Institute and the Freeman Spogli Institute for International Studies, the founding Director at the Center on Food Security and the Environment, and Professor of Economics (by courtesy) at Stanford University. She received her B.A. in Economics and Environmental Studies from the University of Colorado, her M.Sc. in Economics from the London School of Economics, and her Ph.D. in applied economics from Stanford University. Her research focuses on policies and practices to improve global food security and protect the environment on land and at sea. She works with her students in many locations around the world. She has been involved in many field-level research projects around the world and has published widely on issues related to intensive crop production, aquaculture and livestock systems, biofuels, climate change, food price volatility, and food policy analysis. In addition to her many peer-reviewed papers, Naylor has published two books on her work: The Evolving Sphere of Food Security (Naylor, ed., 2014), and The Tropical Oil Crops Revolution: Food, Farmers, Fuels, and Forests (Byerlee, Falcon, and Naylor, 2017).

She is a Fellow of the Ecological Society of America, a Pew Marine Fellow, a Leopold Leadership Fellow, a Fellow of the Beijer Institute for Ecological Economics, a member of Sigma Xi, and the co-Chair of the Blue Food Assessment. David Battisti received a Ph.D. in Atmospheric Sciences (1988) from the University of Washington. He was an Assistant Professor at the University of Wisconsin until 1990. Since then, he has been on the Faculty in the Department of Atmospheric Sciences at the University of Washington, and was the Director of JISAO from 1997-2003. Presently, he is the Tamaki Professor of Atmospheric Sciences at the University of Washington and Director of the University's Earth Initiative.

David Battisti's research is focused on understanding the natural variability of the climate system. He is especially interested in understanding how the interactions between the ocean, atmosphere, land and sea ice lead to variability in climate on time scales from seasonal to decades. His previous research includes coastal oceanography, the physics of the El Nino/Southern Osciallation (ENSO) phenomenon, midlatitude atmosphere/ocean variability and variability in the coupled atmosphere/sea ice system in the Arctic. Battisti is presently working to improve the El Nino models and their forecast skill, and to understand the mechanisms responsible for the drought cycles in the Sahel, and the decade-to-decade changes in the climate of the Pacific Northwest, including how the latter affects the snow pack in the Cascades and coastal ranges from Washington to Alaska. He is also working on the impacts of climate variability and climate change on food production in Mexico and Indonesia.

Battisti's recent interests are in paleoclimate: in particular, the mechanisms responsible for the remarkable "abrupt" global climate changes evident throughout the last glacial period.

Battisti has served on numerous international science panels, on Committees of the National Research Council. He served for five years as co-chair of the Science Steering Committee for the U.S. Program on Climate (US CLIVAR) and is co-author of several international science plans. He has published over 60 papers in peer-review journals in atmospheric sciences and oceanography, and twice been awarded distinguished teaching awards.

Rural farmers in sub-Saharan Africa live under risky conditions. Many grow low-value cereal crops that depend on a short rainy season, a practice that traps them in poverty and hunger.

But reliable access to water could change the farmers' perilous situation. Stanford scientists are calling for investments in small-scale irrigation projects and hydrologic mapping to help buffer the growers from the erratic weather and poor crop yields that are expected to worsen with climate change in the region.

The potential for increased irrigation is there, said Jennifer Burneya fellow at Stanford's Center on Food Security and Environment at the Freeman Spogli Institute for International Studies.

Burney's team partnered with the Solar Electric Light Fund (SELF) to measure economic and nutritional impacts of solar-powered drip-irrigated gardens on villages in West Africa's Sudano-Sahel region. Burney will present the group's work on small-scale irrigation Wednesday, Dec. 7, at the fall meeting of the American Geophysical Union in San Francisco.

"Irrigation is really appealing in that it lets you do a lot of things to break this cycle of low productivity that leads to low income and malnutrition," said Burney.

Modern irrigation often means multi-billion-dollar projects like damming rivers and building canals. But Burney says that these projects have not reached sub-Saharan Africa because countries lack the capital and ability to carry out big infrastructure projects.

A different approach, gaining popularity in sub-Saharan Africa, involves cooperation. Individuals or groups, called smallholders, organize to farm small plots and ensure their access to irrigation. These projects allow farmers to grow during the dry season and produce profitable, high-nutrition crops like fruits and vegetables in addition to the cereal crops they already grow.

Still, only 4 percent of cropland in sub-Saharan Africa is irrigated.

Smallholder irrigation

Burney and her colleagues' work in two northern Benin villages is an example of successful investment in smallholder irrigation. They worked with women's cooperative agricultural groups to install three solar-powered drip irrigation systems. Drip irrigation conserves water by delivering it directly to the base of plants. The technique also reduces fertilizer runoff.

The team surveyed 30 households in each village and found that solar drip irrigation increased standards of living and increased vegetable consumption to the U.S. Department of Agriculture's recommended daily allowance. By selling the vegetables, households were able to purchase staples and meat during the dry season.

Successful smallholder irrigation projects have high investment returns, said Burney. Her team has seen real success from irrigation projects – like those in Benin – that provide enough returns for women to send kids to school or buy small business equipment like a sewing machine or market stall.

"That's when I think it really becomes a ladder out of poverty," Burney said.

Lessons for success

For solar technology projects to be successful, Burney said, just dropping in and giving people irrigation kits doesn't work. Communities need access to a water source and need to see the benefits of a project.

"You need the technology and management and the water access, all together," said Burney. "Our solar project incorporates all of that."

According to Burney, smallholders need not limit themselves to solar irrigation systems. "Solar is great if you have an unreliable fuel," she said. "But if you're someplace that's connected to the grid, an electrical pump would more economical."

"There are a lot of different solutions that involve many different kinds of water harvesting," Burney said. "Groundwater, rainwater, surface water, and there are a lot of places in the Sahel, like Niger, for example, where there are artesian wells." The Sahel is a transition zone between the Sahara Desert and the savannas further south.

Given the diversity of water resources in West Africa, Burney suggests that nongovernmental organizations and governments prioritize detailed hydrologic mapping in the region. Otherwise, the cost of geophysical surveys and finding water sources, especially unseen groundwater, could become an insurmountable barrier for farm communities.

"It needs to be really detailed, comprehensive, usable information that's out there for everybody to be able to take advantage of," she said.

Burney says that both of the benefits that farmers get from irrigation systems –growing outside of the rainy season and producing more diverse, profitable crops – are important for adapting to climate change.

"You can produce more value on less land in most cases and not be as beholden to the whims of the rainy season," she said. Having more disposable income also will reduce vulnerability to hunger and malnutrition. "Economic development can be a form of adaptation," she said.

CDDRL pre-doctoral Alex Ruiz Euler will explore the evolution of inequality at the municipal level in Mexico with original measures constructed using household-level data from 1990, 2000 and 2010. Euler will analyze how increasing electoral competition at both the federal and local level correlates with these patterns of inequality.
 
Speaker Bio:

Alex Ruiz Euler is a 2011-2012 pre-doctoral fellow at CDDRL and a PhD candidate in political science from the University of California, San Diego. His dissertation focuses on the effects of democratization and economic inequality on the provision of education. His case study is Mexico and is developing novel databases for these indicators at the municipal and locality level. He is also part of a collaborative effort to analyze more broadly the relation between governance and the provision of public goods, including water, health and public security.

The November 17 Liberation Technology Seminar was co-hosted by the Stanford Institute for Economic Policy Research (SIEPR), a nonpartisan economic policy research organization that unites remarkable economic talent from all parts of Stanford University. This seminar featured four student-led design projects that were created in the Designing Liberation Technologies course taught each spring at Stanford's Hasso Plattner Institute of Design (d.school) by professors' Josh Cohen and Terry Winograd. The class works in partnership with the University of Nairobi's School of Computing and Informatics to develop user-based designs that address outstanding challenges in the slums of Nairobi, Kenya. 

Josh Cohen described the three main elements of the design school class:

(1) There is a focus on user/human-centered design. Projects are designed based on a close engagement with end users: the people who will be using the solutions. (2) Courses are inter-disciplinary. The idea is to take people who are deeply embedded in a certain discipline and put them together with people from other disciplines. In order to come up with a solution to a real problem, team collaboration is necessary and (3) The approach to problem solving is about practice, not about theory. It is necessary to fail early and fail often in the process of creating workable solutions.

The panelists consisted of students who were working with partners in Kenya on how mobile technology could be used to address various social issues. A multidisciplinary team of students work in close collaboration with partners in Kenya, and some of these ideas are now being tested on the ground.

The first project that was described was M-maji (mmaji.wordpress.com). Sangick Jeon described how water in Kibera is scarce, costly and contaminated. Their aim is to use the nearly ubiquitous mobile technology to improve access to water. Vendors are able to advertise their water and then buyers can ask questions and provide feedback. This saves the time and resources that it takes to find clean water.

Nishuari, the second project, addresses the problem of inaccurate advice and rumors which lead to risky sexual behavior and often HIV and AIDS. They provide a mobile counseling service whereby individuals can submit health questions and receive responses from trained counselors using text messages. This helps to break down logistical and social barriers.

The third, Makmende considered the issue of serious crime in Kibera. Their aim is to look at how individuals can get safely from Point A to Point B. They set up walking groups so that people would not have to walk alone. Over 90% of people had a mobile phone and this was seen to be an effective way of building timely communication, so that groups of people can coordinate to walk more safely to reach their destinations.

The final project was Take Taka, which was the one project that was not focused on using computer technology. The team took up sanitation as the project and decided early on that the use of mobile phones adds no value to the issue. The focused instead of designing a special sanitary bucket that could be used instead of a toilet inside the home, and on developing a system of transmitting the deposits to a biogas unit. This project however has not been carried forward.

Congratulations to Anand Habib, selected this weekend for a Rhodes Scholarship. Habib, 22, of Houston, Texas, is a 2011 graduate of Stanford, where he earned a bachelor's degree in biology, with honors in international security studies. He plans to pursue a master's degree in public policy and in medical anthropology at Oxford.

Habib is currently working on community health programs at St. Joseph's Clinic in Thomassique, Haiti, under a one-year global health fellowship awarded by Medical Missionaries. The nonprofit organization is a volunteer group of more than 200 doctors, nurses, dentists, and others who work to improve the health of the poorest of the poor in the United States and throughout the world.

In 2011, he won a Deans' Award for Academic Accomplishment which honors extraordinary undergraduate students for "exceptional, tangible" intellectual achievements. One of the professors who nominated him for the award described him as a "superb critical thinker" whose work is characterized by "creative genius" and "mature insights," adding that he "exemplifies exactly the kind of deeply informed, pragmatic and caring leadership that the world needs and Stanford enables."

As a Stanford student, Habib worked on behalf of politically and medically disenfranchised people in India, Mexico and Guatemala. On campus, he turned the Stanford tradition of the annual Dance Marathon into a vehicle dedicated to addressing the HIV/AIDS pandemic by engaging not only Stanford students but also local communities and corporations, raising more than $100,000. His exceptional work was recognized by his participation in the Clinton Global Initiative University Conference in April, 2011.

From Haiti, Habib discussed his current position, his plans in Oxford, and how his CISAC thesis relates to his work. 

What are you currently working on? 

I am currently working on a global health fellowship with an NGO that operates a clinic in Haiti's Central Plateau. As part of the fellowship I am helping to manage a number of community and public health projects including fortified salt (to counter iodine deficiency and filariasis), a point-of-use potable water system, a malnutrition intervention program, and a small system of community health committees and community health workers. For the past two months, I've taken on a number of administrative functions along with my co-fellow, who is also a recent college graduate. I started the fellowship shortly after graduation at the end of June 2011 and I am slated to be in Haiti until the end of June 2012.

What will you do at Oxford?

I proposed two one-year Masters programs: a masters in public policy from Oxford's new Blavatnik School of Government, and an M.Sc. in medical anthropology. As of now, I'm thinking of sticking with my proposed programs -- probably pursuing the medical anthropology degree first as it is a bit more theoretical. I think it would be better to have the more "academic" experience first before gaining the practical tools that an MPP will hopefully afford me. After a short send-off in Washington, D.C., at the end of September 2012, I will fly with the other American Rhodes Scholars-elect to Oxford the first week of October with term beginning shortly thereafter.

How does your work in the CISAC Honors program relate to what you are currently doing, your plans for Oxford, or what you'll be doing afterward? 

My honors thesis has allowed me to contextualize my experiences in Haiti in some fairly amazing ways. Two very persistent issues in Haiti are that of poor governance -- e.g. lack of strong regulatory structures, lack of government effectiveness, etc. -- and ineffective aid modalities, or the ways in which aid is delivered. My thesis was situated exactly at this nexus: how to deliver aid more effectively in order to improve governance. Ninety-nine percent of relief aid after the 2010 earthquake in Haiti was funneled to non-state organizations and only 12 percent of recovery aid went toward supporting government activities. In Haiti, I've witnessed how detrimental a patchwork system of NGO activity can be with little coordination between NGOs and little harmonization of NGO activities with government objectives. It becomes very frustrating at times, because, if my thesis is any indication, things need not be this way if both NGOs and states were engaged in collaborative partnerships -- along the lines of what Dr. Paul Farmer, deputy U.N. special envoy to Haiti, calls "accompaniment."

My initial interest in the topic of global health financing was spurred by work out of Oxford's Global Economic Governance Project. The director of the project, Dr. Ngaire Woods, was actually named the Dean of the Blavatnik School of Government through which the MPP is offered. I'd love to continue exploring the issues broached in my thesis at Oxford -- especially issues of aid effectiveness and sustainable systems of delivering development aid for health. In the future, I hope to become a physician-policymaker, helping to not only deliver care to individuals in difficult conditions around the world, but also in fostering more robust global health governance.

Promotion of smallholder irrigation is cited as a strategy for enhancing income generation and food security for sub-Saharan Africa’s poor farmers, but what makes this technology a successful poverty alleviation tool? In the short run, the technology should pave the way for increased consumption, asset accumulation, and reduced persistent poverty among users. Over the longer run, it should lead to institutional feedbacks that support sustained economic development and nutritional improvements. Our conceptual model and review of case studies reveal the importance of three sub-components of irrigation technology—access, distribution, and use—and the ways in which the design of the technology itself can either bridge, or succumb to, institutional gaps. These critical features are illustrated in an experimental evaluation of a solar-powered drip irrigation project in rural northern Benin, which provides a controlled study of technology impacts in the Sudano-Sahel. The combined evidence highlights the technical and institutional requirements for project success and points to two important areas of research in the scale-up of any small-scale irrigation strategy: the risk behavior of water users, and the evolution of institutions that either support or obstruct project replication over space and time.
