China’s rapid development and urbanization have induced large numbers of rural residents to migrate from their homes to urban areas in search of better job opportunities. Parents typically leave their children behind with a caregiver, creating a new, potentially vulnerable subpopulation of left-behind children in rural areas. A growing number of policies and nongovernmental organization efforts target these children. The primary objective of this study was to examine whether left-behind children are really the most vulnerable and in need of special programs. Pulling data from a comprehensive data set covering 141,000 children in ten provinces (from twenty-seven surveys conducted between 2009 and 2013), we analyzed nine indicators of health, nutrition, and education. We found that for all nine indicators, left-behind children performed as well as or better than children living with both parents. However, both groups of children performed poorly on most of these indicators. Based on these findings, we recommend that special programs designed to improve health, nutrition, and education among left-behind children be expanded to cover all children in rural China.

A Global Perspective on Food Policy

I applaud Mark Bittman, Michael Pollan, Ricardo Salvador, and Olivier de Schutter for advocating the introduction of a national food policy in the U.S. Greater emphasis in our current farm legislation on nutrition, health, equity, and the environment is clearly warranted and long overdue. As the authors note, Americans’ access to adequate nutrition at all income levels affects educational and health outcomes for the nation as a whole. Poor nutrition thus plays a role in determining the level and distribution of economic and social wellbeing in the U.S, now and in the future. It is surprising that no one within the large circle of Presidential hopefuls has raised the topic of food, not just agriculture, as a major political issue for the 2016 election.

The U.S. is not unique. Virtually every country with an agrarian base has, at some point in history, introduced agricultural policies that support farmers and provide incentives for them to produce major commodities. At the time, governments have been able to justify these policies on several grounds: national security (avoiding excess dependence on foreign nations for food), economic growth (using agricultural surpluses as an engine of economic growth), and social stability (keeping its population well-fed to avoid social unrest). Once agricultural policies are implemented, they typically give rise to institutions and vested political interests that perpetuate a supply-side orientation to food and agriculture. In the U.S., the political institutions that govern food and agriculture have their roots in historical political precedents that date back to the 1860s, and later to the 1930s when the New Deal was promulgated. Farm interests have been entrenched in the U.S. political system for quite some time, and they cannot be easily removed.

There is a general rule for successful policies: Align incentives with objectives. A corollary to this principle is that objectives change over the course of economic development. For the United States in earlier eras, and for many developing economies in recent decades, meeting basic calorie needs has been the first order of business. This objective has been largely achieved through public investments in infrastructure (irrigation, roads), research and development, commodity support programs, incentives for private agribusiness development, and other supply-side measures.

With successful agricultural growth and rising incomes, many countries face a new set of food and nutrition challenges: eliminating “hidden hunger” (deficiencies in iron, vitamin A, calcium, zinc and other micronutrients), and abating the steady rise in obesity that results from a transition to diets rich in energy-dense carbohydrates, fats, and sugar. Hidden hunger affects some three billion people worldwide. It is prevalent among low-income households in almost all countries, impairs cognitive and physical development (especially among infants up to two years of age) and thus limits a nation’s educational and economic potential. Meanwhile, rates of obesity now surpass rates of energy-deficient hunger throughout the world, even in developing nations.

The objectives of food and agricultural policies in virtually all countries need to shift, on balance, from promoting staple food supplies to enhancing nutrition. I am not suggesting an abandonment of agriculture, but rather an enrichment of agriculture with more crop diversity to support the nutritional needs of all people. If improved nutrition is the objective, what are the correct incentives? Proper incentives will differ among countries, but will inevitably require a fundamental change in institutional structure. With a shift from supply- to demand orientation, there needs to be a transition from Ministries of Agriculture to Ministries of Food. After all, the main goals of a Ministry of Agriculture are to increase the volume of agricultural production and to improve economic growth in the agricultural sector. The main goal of a Ministry of Food, by contrast, is to enhance the nutrition and food security of the entire population.

Bittman, Pollan, Salvador, and de Schutter emphasize that replacing the U.S. Department of Agriculture (USDA) with a “U.S. Department of Food, Health, and Wellbeing” would be difficult at best. It would require unprecedented political will and cooperation among parties. The same can be said for institutional change in agricultural ministries throughout the world. Regardless of the challenges, however, nothing will change until the conversation surrounding food policies, politics, and institutions takes a major turn.

More than 46 million Americans live in poverty and high rates of food insecurity and obesity are also a persistent concern. The Supplemental Nutrition Assistance Program (SNAP), formerly known as food stamps, occupies a central role in the U.S. safety net as the only universal aid program for low income individuals. In this talk, Professor Hilary Hoynes will review the evidence on the two  goals of SNAP: providing income support and improving nutrition. Professor Hoynes will discuss the trends in poverty and inequality in the U.S, and how SNAP affects poverty overall and particularly in the Great Recession. Additionally, she will review the evidence on the impact of SNAP on food insecurity and health. This will include new evidence on how access to social safety net programs in early life affect health and human capital outcomes in adulthood. 


Hilary Hoynes is a Professor of Public Policy and Economics and holds the Haas Distinguished Chair in Economic Disparities. She is the co-editor of the leading journal in economics, American Economic Review. Hoynes received her undergraduate degree from Colby College and her PhD from Stanford University.

Hoynes is an economist and specializes in the study of poverty, inequality, and the impacts of government tax and transfer programs on low income families. Current projects include evaluating the impact of the Great Recession across demographic groups, examining the impact of Head Start on cognitive and non-cognitive outcomes, examining the impact of the Earned Income Tax Credit on infant health, and estimating impacts of U.S. food and nutrition programs on labor supply, health and human capital accumulation.

In addition to her faculty appointment, Hoynes has research affiliations at the National Bureau of Economic Research, the UC Davis Center for Poverty Research and the Institute for Fiscal Studies. She sits on the Advisory Board of the Stanford Institute for Economic Policy Research and previously has sat on the National Advisory Committee of the Robert Wood Johnson Foundation Scholars in Health Policy Research Program and the Advisory Committee for the National Science Foundation, Directorate for the Social, Behavioral, and Economic Sciences. Prior to joining the Goldman School she was a Professor of Economics at UC Davis.

David Studdert and colleagues explore how to balance public health, individual freedom, and good government when it comes to sugar-sweetened drinks. Over the last decade, many national, state, and local governments have introduced laws aimed at curbing consumption of sugar-sweetened beverages (SSBs), especially by children. The main regulatory approaches are taxes, restrictions on the availability of SSBs in schools, restrictions on advertising and marketing, labeling requirements, and government procurement and benefits standards. Efforts to regulate in this area often encounter stiff opposition, including claims that the laws are inequitable, do not achieve their goals, and have negative economic effects. Several lessons can be drawn from the international experience with SSB regulation to date, which may inform future design and implementation of legal interventions to combat noncommunicable disease.

Rates of obesity in the United States remain extremely high. New statistics show that nearly two-thirds of adults are at an unhealthy weight and that – for the first time ever – obese Americans now outnumber those who are merely overweight.

Two Stanford public health law experts say one of biggest culprits of the obesity epidemic – on top of fast foods and our sedentary lifestyle – are sugary drinks.

And they believe the sweet spot for public health law in curbing the adverse effects of sugar-sweetened beverages (SSBs) lies in the strategic use of measures such as higher SSB taxes, limits on advertisements targeting kids, and restrictions on soft drinks and sugar-sweetened teas and sports drinks in government institutions, such as public schools.

“It’s always possible to get more and better evidence about the effectiveness of public health laws,” says David Studdert, a professor of medicine at the Stanford School of Medicine, professor at the Stanford Law School and core faculty member at the Center for Health Policy/Center for Primary Care and Outcomes Research.

“But enough is already known about the promise of some legal interventions to curb SSB consumption – significant tax hikes and advertising restrictions are two good examples – to be fairly confident that they would make a difference.”

Studdert is the lead author of a review paper published July 7 in PLoS Medicine, entitled, “Searching for Public Health Law’s Sweet Spot: The Regulation of Sugar-Sweetened Beverages.”

Studdert and senior author Michelle Mello, professor of law and professor of health research and policy at the School of Medicine, and co-author Jordan Flanders, a former Stanford Law School student, argue that sugary drinks are a substantial, yet preventable contributor to the global burden of obesity and associated health conditions.

A new study published June 29 in the American Heart Association journal Circulation linked the consumption of sugary drinks to an estimated 184,000 adult deaths each year, with more than 25,000 of those Americans. The study, conducted by researchers from Tufts University, found that the beverages are responsible for an estimated 133,000 of those deaths from diabetes, 45,000 from cardiovascular disease and 6,450 from cancer.

While Americans’ consumption of sugary drinks has plateaued, according to the Tufts study, about three-fourths of the deaths due to SSBs are now in developing countries. Mexico leads with 24,000 total deaths. The United States still ranks fourth, however, just behind South Africa and Morocco.

The Stanford researchers say the evidence shows that sugary drinks are contributors to the global obesity epidemic, but the appropriate reach of regulation to curtail SSB consumptions remains highly contested.

“Finding public health law’s sweet spot requires regulatory approaches that are capable both of achieving measurable improvements to public health and of winning victories in courts of law and public opinion,” the researchers write.

But efforts to regulate the drinks often encounter stiff opposition, including claims that the laws are inequitable, do not achieve their goals, and have negative economic effects.

New York City’s attempt to ban the sale of jumbo-sized sugary drinks sold in city restaurants, theaters and food carts triggered international headlines and a firestorm of opposition. The soft drink industry embarked on a multimillion-dollar campaign to block the proposal championed by former Mayor Michael Bloomberg.

The proposal died last year when the New York State Court of Appeals ruled that the city’s Board of Health had “exceeded the scope of its regulatory authority.”

Taxes on SSBs, the most commonly adopted measure, vary widely, the authors write. A few countries, most notably several South Pacific island nations, where obesity rates are among the highest in the world, have introduced very high taxes on sugary drinks.

But most sugar-sweetened beverage taxes add between 5 and 9 cents per liter. This is well short of the level that experts argue is needed to significantly affect consumption and weight outcomes: a sales tax of at least 20 percent of the container’s price or a specific excise tax of 1 cent per ounce.

“In the United States, there have been many government proposals to introduce or raise taxes – most unsuccessful,” the authors write. “The beverage industry has invested heavily in public relations firms and `grassroots’ organizations to oppose the initiatives.”

Berkeley, Calif., recently became the first U.S. city to pass an SSB tax, a penny-per-ounce excise on soda distributors, but a similar ballot measure in nearby San Francisco failed. At least 22 states have proposed SSB taxes since 2010, but only one state, Washington, passed a measure at the level recommended by economists – and it was repealed the following year in a voter referendum.

Yet U.S. childhood obesity has more than doubled in children and quadrupled in adolescents in the past 30 years, according to the Centers for Disease Control and Prevention. More than one-third of children and adolescents are overweight or obese.

“There is broad consensus in the public health community that reducing the influence of advertising is a critical step in addressing the spread of childhood obesity,” the authors say.

The United States and Canada have sought to regulate advertisers through a soft approach — mainly via voluntary guidelines and pressure to self-regulate, the authors write.

“These appear to have had only a modest impact on marketing practices,” they said. “U.S. regulators face considerable legal barriers in going further, including courts’ increasingly expansive interpretations of the scope of protected commercial speech under the First Amendment. Unless judicial currents shift, it will remain extremely difficult to impose restrictions on SSB advertising.”

Mello said low- and middle-income countries should anticipate that SSB companies will increasingly target them as promising markets, and that those developing countries should start crafting their regulatory responses now.

“Our experience with tobacco control teaches us that lower- and middle-income countries need to become wary when product regulation in the U.S. tightens,” Mello said. “Like squeezing a balloon, it pushes companies to intensify their marketing efforts overseas, and our public health problems get exported."

And, the authors note, while policy nudges have become fashionable, “there are dangers in treading too lightly.” “Strategies such as calorie labels, portion caps, and small beverage taxes preserve consumer freedom but are typically too modest to affect consumer behavior – and such modesty can be recast as arbitrariness. Industry opposition will come whether the intervention is modest or aggressive but should be easier to combat if officials can show their policy is effective,” they wrote.

“One somewhat surprising message that comes from reviewing how courts have handled challenges to SSB laws is that regulators can run greater risks of having their laws struck down if they are too timid,” Studdert said.

“Courts weigh effectiveness, and modest attempts to change behavior are often ineffective,” he said. “So one piece of advice regulators in this area should consider is to ‘go big or go home’.”

Soil-transmitted helminths (STHs) are parasitic intestinal worms that infect more than two out of every five schoolchildren in rural China, an alarmingly high prevalence given the low cost and wide availability of safe and effective deworming treatment. Understanding of local knowledge, attitudes, and practices regarding STHs in rural China has until now, been sparse, although such information is critical for prevention and control initiatives. This study elucidates the structural and sociocultural factors that explain why deworming treatment is rarely sought for schoolchildren in poor villages of rural China with persistently high intestinal worm infection rates. In-depth, qualitative interviews were conducted in six rural villages in Guizhou Province; participants included schoolchildren, children’s parents and grandparents, and village doctors. We found evidence of three predominant reasons for high STH prevalence: lack of awareness and skepticism about STHs, local myths about STHs and deworming treatment, and poor quality of village health care. The findings have significant relevance for the development of an effective deworming program in China as well as improvement of the quality of health care at the village level.

A new study by Center on Food Security and the Environment researchers finds that smallholder irrigation systems - those in which water access (via pump or human power), distribution (furrow, watering can, sprinkler, drip lines, etc.), and use all occur at or near the same location - have great potential to reduce hunger, raise incomes and improve development prospects in an area of the world greatly in need of these advancements. Financing is crucial, as even the cheapest pumps can be prohibitively expensive otherwise.

These systems have the potential to use water more productively, improve nutritional outcomes and rural development, and narrow the income disparities that permit widespread hunger to persist despite economic advancement. Only 4 percent of agricultural land in sub-Saharan Africa is currently irrigated.

"Success stories can be found where distributed systems are used in a cooperative setting, permitting the sharing of knowledge, risk, credit and marketing as we've seen in our solar market garden project in Benin," said Jennifer Burney, lead author of the study published in the Proceedings of the National Academy of Sciences.

Moving forward development communities and sub-Saharan African governments need a better understanding of present water resources and how they will be affected by climate change.

"Farmers need access to financial services—credit and insurance—appropriate for a range of production systems," said co-author and Stanford Woods Institute Senior Fellow Rosamond Naylor. "Investments should start at a smaller scale, with thorough project evaluation, before scaling up."

FSE continues to contribute to these evaluations and added eight new villages to our project in Benin last year.

Although China has experienced rapid economic growth over the past few decades, significant health and nutritional problems remain. Little work has been done to track basic diseases, such as iron-deficiency anemia, so the exact prevalence of these health problems is unknown. The goals of this study were to assess the prevalence of anemia in China and identify individual, household and community-based factors associated with anemia. We used data from the 2009 China Health and Nutrition Survey (CHNS), including the measurement of he- moglobin levels among 7,261 individuals from 170 communities and 7 provinces in central and eastern China. The overall prevalence of anemia was 13.4% using the WHO’s blood hemoglobin thresholds (1968). This means in China’s more developed central and eastern regions up to 180 million people may be anemic. Some vulnerable subgroups were disproportionately affected by anemia. Seniors (aged 60 years and above) were more likely to be anemic than younger age cohorts, and females had higher anemia prevalence among all age groups except among children aged 7 to 14 years. We found a negative correlation between household wealth and the presence of anemia, suggesting anemia prevalence may decline as China’s economy grows. However, the prevalence of anemia was greater in migrant households, which should be experiencing an improved economic status.

The study provides evidence that a country’s ability to reduce the gap in child-mortality rates is related to good governance.

The child-mortality gap has narrowed between the poorest and wealthiest households in a majority of more than 50 developing countries, a new study from the Stanford University School of Medicine has found.

This convergence was mostly driven by the fact that child-mortality rates declined the fastest among the poorest families. In the countries where the gap increased, the study identified a common thread: poor governance.

The findings provide important information for making decisions about prioritizing global health investments to effectively promote equity, said Eran Bendavid, MD, assistant professor of medicine, core faculty member at CHP/PCOR, and the study’s author.

The study, published online Nov. 10 in Pediatrics, analyzed data from nearly 1 million families living in 54 low- and middle-income countries to determine the relationship between mortality in children under the age of 5 and wealth inequality.

“In many countries, national wealth has increased hand-in-hand with increasing health inequality. That’s been a signature of our time,” Bendavid said. “It’s a pressing concern for many societies, especially in wealthy countries, but it’s also been an issue in low- and middle-income countries.”

Assessing child mortality within developing countries

Many studies have assessed the national child mortality trends in developing countries, but they say little about the mortality gap between the poorest and wealthiest within those countries. National trends could be associated with either narrowing or widening gaps between the poorest and wealthiest populations, Bendavid noted. For example, if child mortality decreases faster among the wealthy compared with the poor, the overall child-mortality rate in that country could decrease even as the mortality gap widens. Alternatively, if child mortality decreases faster among the poor, the health gap could narrow.

To fill this gap in knowledge, the study sought to understand whether developing countries are experiencing a widening or narrowing mortality-rate gap among children under 5 of the poorest and wealthiest families.

To compare wealth status and under-5 child mortality within a country, Bendavid used data from the demographic and health surveys for 1.2 million women living in 929,224 households in 54 developing countries. The women provided information about their children’s survival status. 

 “The people who conduct these surveys, they’re intrepid surveyors,” said Bendavid, who is also a core faculty member of Stanford Health Policy, which is part of the Freeman Spogli Institute for International Studies. “They reach remote villages up the Congo basin and in the Sahel in Niger, and track the heads of households and women for these in-depth interviews.”

The surveys include information about each woman’s birth histories, including detailed birth registries documenting millions of children. With this information, Bendavid could estimate the probability of a child dying before reaching age 5 per 1,000 live births.

Tallying household possessions

Determining each household’s wealth status was not as straightforward as reviewing annual income and tax returns, which don’t exist in the countries involved in the study. “These surveys tally the possessions in the household. What is the floor made of? What is the roof made of?” Bendavid said. “You can get a wide distribution of household possessions that reflects to a large degree the household wealth.”

Next, Bendavid developed a three-tier wealth index using the household assets. The three wealth categories were relative — poorest, middle and wealthiest.

To analyze trends in wealth status and under-5 mortality, Bendavid looked at all developing countries that had completed the surveys in two specific time frames: 2002-07 and 2008-12. The study found that the under-5 mortality rates among the poorest groups had decreased the most rapidly. The average decline was 4.36 deaths each year per 1,000 live births among the poorest, 3.36 among the middle and 2.06 among the wealthiest. Because the poorest group’s mortality rate is decreasing more quickly that the other groups, the gap in child-mortality rates is closing.

This is good news, Bendavid said. However, not all countries followed this same trend. In a quarter of the surveys examined by the study, inequality in under-5 mortality increased over time.

Bendavid found that four factors were present in countries with a narrowing child-mortality gap: government effectiveness, rule of law, control of corruption and regulatory quality. He found that the difference in mortality rates was significantly associated with the governance score: Better governance scores were related to greater convergence in mortality rates among the three wealth groups.

Benefits from controlling communicable diseases

Bendavid said the evidence in this study is consistent with gains in controlling communicable diseases, such as malaria, measles, diarrhea and respiratory illnesses, that preferentially affect the poorest. Over the past decade, international health aid organizations have financed interventions for these diseases at a high rate.

It makes a persuasive case that these improvements have often begun to benefit the poor even more than the better-off.

“Dr. Bendavid’s study is an important contribution to knowledge about child health improvements in the developing world,” said Davidson Gwatkin, a senior fellow at the Results for Development Institute and a senior associate at Johns Hopkins Bloomberg School of Public Health. “It makes a persuasive case that these improvements have often begun to benefit the poor even more than the better-off.” Gwatkin was not involved in the study.

The study also raises questions about the role of foreign aid institutions in low- and middle-income countries. While the aid efforts are making a difference in child-mortality rates in countries with effective governments, the study seems to show that this is not the case in nations with poor governance, Bendavid said.

“We have the technologies, we have the means, we have the know-how to reduce child mortality dramatically,” said Bendavid. “Even for such low-hanging fruit, however, implementation is not always easy.
