Most discussions about health care reform focus on how to achieve coverage for all (or at least most) people. There is little realistic attention given to the "morning after problem": assuming everyone were "magically" covered, why would we expect the system to deliver high quality care in an efficient manner when it clearly does not do so now? If we think more carefully about how care should be delivered, paid for, and ultimately, financed in order to achieve high value, we may generate a proposal with the political momentum for fundamental change. Such change, however, must recognize that the current interest groups will have "seats at the table" during the legislative process.
